Best way to track spell results?

I want a spell log that is useful later, not just pretty. What fields should every entry include?

Purpose, date, moon phase, ingredients, wording, energy before and after, and a follow-up date. I also add a notes line for mundane actions.

The follow-up date is the piece I always forget. Without that, I only remember the dramatic workings and lose the quiet useful ones.
